  • You will be directed to the tournament page after completion.
  • Please make your payment online from the tournament listing or
    mail your payment to the address provided.
    Check must be received three weeks prior to the tournament.
    Inside of three weeks to the tournament, online payment is required.
  • Your team will be entered into the tournament once payment is received.
  • Registration will close once the maximum field is reached regardless of the date.
  • All online entries agree to waiver presented.
  • All participants and guardians MUST sign and then submit the OnTurf Sports waiver and roster form.
  • All teams MUST have proof of team insurance.

I (we) do hereby release, absolve, indemnify, and otherwise hold blameless ON TURF SPORTS LLC, the organizers, sponsors, directors and game officials of any damages, demand's or actions whatsoever, which may arise out of our participation in this ON TURF SPORTS LLC event. The team players should not participate unless they are medically and physically able to do so and it is their sole responsibility to determine if they are medically and physically able to participate in the event. Our team understands the nature of the event and our team ASSUMES ALL RISKS associated with our voluntary participation in this event including, but not limited, contact with other participants or objects and the effects of the weather, including extreme temperatures and precipitation. Knowing these facts, I, for myself, our team, their heirs, executors, administrators, or anyone else who might make a claim on our behalf, COVENANT NOT TO SUE, WAIVE ALL CLAIMS AGAINST, AND FULLY RELEASE AND DISCHARGE OnTurf Sports LLC, or its employees, any and all sponsors affiliated with the event, workers, and volunteers, for ANY AND ALL claims or liability, whether foreseen or unforeseen, for death, personal injury (whether permanent or temporary), or property damage arising out of, or in the course of, our participation in this event. I further grant full permission to OnTurf Sports LLC and/or agents authorized by them, to use any photographs, video, or other record of the event for any reasonable purpose.
