2010 Tournament Registration/Roster Form and Waiver

Select, print, complete and return the signed registration form along with the required entry fee to OnTurf Sports LLC or register with BPA or NSA directly by selecting on the tournament(s) of interest below. A copy of your team's proof of insurance and the tournament registration/roster form and waiver (above right) must be filled out and returned with payment to OnTurf Sports at least two weeks before the tournament begins.
